How to Choose the Right Orthotic: A Foot Surgeon’s Decision Guide
The orthotic market is crowded with options at every price point and with every claim imaginable. Most people navigating this landscape do not have a clinical framework for distinguishing between products — they are choosing based on marketing language, price, or what worked for a friend with a different foot condition.

The Core Design Variables That Matter
Rigidity determines mechanical correction capacity. Soft orthotics provide cushioning but minimal motion control. Semi-rigid orthotics — the clinically relevant tier for most biomechanical conditions — provide meaningful resistance to subtalar pronation while maintaining enough compliance for comfort in daily use.
Arch geometry determines contact surface and load distribution. An orthotic with appropriate arch height provides full plantar contact that distributes load across the entire medial arch. Too-high geometry creates painful pressure; too-low geometry provides no functional support.
Length determines which regions of the foot are addressed. Full-length orthotics extend from heel to toe and influence both rearfoot and forefoot mechanics. Three-quarter length orthotics fit more easily in dress footwear.
Matching Orthotic to Condition
Plantar fasciitis: Semi-rigid full-length orthotic with medial arch support and rearfoot posting. The arch support reduces fascial tension; the rearfoot posting limits the excessive pronation that increases fascial load.
Flat feet / adult-acquired flatfoot: Semi-rigid or rigid orthotic with medial arch support and rearfoot control. Motion-control footwear paired with an appropriate orthotic limits progressive arch collapse. Softer orthotics are inadequate for this presentation.
Metatarsalgia / forefoot pain: Full-length orthotic with a metatarsal pad positioned just proximal to the metatarsal heads, shifting load from the metatarsal heads to the metatarsal shafts.
High arches / cavus foot: A full-length cushioned orthotic with lateral forefoot posting to account for the rigid, supinating foot pattern. Standard arch-support orthotics designed for flat feet are not appropriate for cavus foot presentations.
When OTC Orthotics Are Appropriate vs. Custom
OTC semi-rigid orthotics are appropriate for the majority of people with common biomechanical conditions. They should be the first orthotic trial before custom orthotics are considered, both for cost efficiency and because most people achieve adequate benefit from this tier. Custom orthotics are appropriate when OTC trials have produced inadequate response or when foot architecture is atypical.
When to See a Foot and Ankle Specialist
- Foot pain that has not responded to 6 weeks of OTC orthotic use with appropriate footwear
- High-arched foot with recurrent ankle sprains or lateral forefoot pain — cavus foot management benefits from specialist orthotic evaluation

Most semi-rigid orthotics require a break-in period of 1 to 2 weeks during which the foot adapts to the new mechanical demands. Begin with 2 to 3 hours per day and increase gradually. Sharp pain over a bony prominence, new numbness, or skin irritation from orthotic contact warrants evaluation — these are not normal break-in sensations.
How do I know when my orthotics need replacing?
Semi-rigid OTC orthotics typically maintain structural integrity for 12 to 18 months of daily use. Signs of needed replacement include visible arch compression or flattening, loss of cushioning in the top cover, or return of symptoms that had been controlled.
